FORM Campus Is Opening a 23,000-Square-Foot Medical Campus at Yonge and Eglinton
Midtown Toronto is getting a large new medical facility at Yonge and Eglinton this October. FORM Face + Body will open FORM Campus at 90 Eglinton Avenue East on Wednesday, October 14, 2026.

The 23,000-square-foot campus brings five physician-led clinics into one address. Services span plastic surgery and aesthetics, hair restoration, men’s health, women’s health, and longevity care.

Five Clinics Share One Address
FORM Campus brings five areas of care into one facility. The concept places several medical and aesthetic services in the same building rather than across separate offices.
FORM Face + Body covers cosmetic surgery, non-surgical injectables, skin treatments and laser services. FORM Hair focuses on hair preservation, transplant procedures and restoration options.
The campus also includes physician-led programs for men’s and women’s health. The women’s clinic, operated with Élevé, lists pelvic health, incontinence, sexual wellness and hormone-related care among its services.
FORM Life+ focuses on healthy aging and longevity care. The clinic says it offers individual treatments, health assessments and annual membership options.
These are medical and wellness services, so treatment suitability depends on an individual consultation. The opening event introduces the facility and its teams rather than replacing clinical assessment.
Inside The 23,000-Square-Foot Facility
The new space is more than a collection of consultation rooms. FORM’s facilities page says the on-site surgical centre has three operating rooms, along with dedicated areas for consultation and recovery.
During Orientation Day, guests can walk through clinical areas that are normally part of patient visits. The FORM Hair area will also show its ARTAS robotic hair-restoration system, according to the official RSVP page.

Meet The Physicians Leading The Campus
Physicians working across surgery, urology, facial plastic surgery and longevity care lead the five clinics.
FORM founder Dr. Ron Somogyi is a plastic surgeon. Dr. Cory S. Goldberg and Dr. Michael Brandt also lead parts of FORM’s longevity and surgical programs.
Dr. Noah Stern directs the men’s health program, while Dr. Alaina Garbens leads FORM Women’s Health by Élevé. The release says all five physicians will be available for interviews during the opening.
